We are offering a $5,000 Sign-On Bonus (to be paid throughout the first year of employment).

Starts at an annual salary of $83,158.40.

The Physical Therapist provides quality Physical Therapy services including resident/patient/client assessment, evaluation, care planning, treatment and consultation in an interdisciplinary environment at Lutheran SeniorLife facilities/agencies including, as assigned, skilled nursing facilities, LIFE programs, home health and/or outpatient clinics.

This position supports LIFE of Butler County and its Participants primarily throughout Butler and the surrounding areas.

Evaluates residents/patients/clients (upon physician referral), establishes the Physical Therapy plan of care, initiates treatment and modifies the plan of care as necessary, according to the principles and practices of Physical Therapy. Completes assessments or screens all residents/patients/clients of the facility/agency on admission and at regular intervals to identify the need for skilled intervention. Completes all documentation and record keeping related to the provision of Physical Therapy services in a timely and accurate manner, following established guidelines. Participates in the interdisciplinary care plan process and communicates to residents/patients/clients, families, caregivers and other health care professionals regarding the coordination of care and discharge planning. Participates in the operation of the Rehabilitation department according to department policies and procedures, i.e., infection control, safety. Complies with all facility/agency policies & procedures. Contributes to the planning, implementation, evaluation and quality management of clinical, department and facility/agency programs. Participates in in-service programming and provides appropriate consultation and training to facility/agency staff. Participates in internal caseload development and, as requested, external marketing activities. Directs and supervises the clinical performance of assigned assistants, techs and students. Complies with Commonwealth of Pennsylvania Physical Therapy Practice Act and all federal and state regulations applicable to the assigned site -Skilled Nursing facilities, PACE/LIFE programs, Home Health programs, Outpatient Clinics. Attends Rehab department meetings and participates in all mandatory in-service programs specific to the Rehab. Performs other duties as assigned.

Bachelor's degree from an accredited four year college or university with a degree in Physical Therapy; masters or doctoral preferred. One (1) to two (2) years' related experience and/or training; or equivalent combination of education and experience. Experience working with senior population. Working knowledge of computer software required for documentation and communication. Maintains current PA license as Physical Therapist including meeting continuing education requirements.
