Clinicians working in home health are on the front lines of health care and often practice at the top of their license. Responsibilities Perform assessments of patient physical, functional, cognitive, and communication levels within the scope of Occupational TherapyDevelop a plan of care with patient goalsProvide evidence-based direct intervention with patients in their place of residencePerform Home Safety EvaluationsCommunicate effectively and respectfully with a diverse patient population and co-workersEducate patients and family members in home programs, use of the equipment and available resourcesSubmit required documentation in a timely mannerQualificationsActive State Occupational Therapist License Current CPR with BLS cardValid driver's license, at least state minimum auto insurance, and an operational vehiclePossess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operationsGreat organization skills and an ability to work with minimal supervisionCapacity to appropriately supervise othersAbility to remain calm in emergency or crisis situationsComputer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) systemPhysical RequirementsDuties require extensive standing, walking, and sittingRequires lifting, positioning, pushing, and/or assistance with transferring patientsRequires frequent reaching, stooping, bending, kneeling, and crouchingThe above statements are only meant to be a representative summary of the major duties and responsibilities performed by the employee in this job.
