Qualifications:
Graduate from an accredited Licensed Practical nursing program.
Current Massachusetts Licensed Practical Nurse Licensure.
One or two years of previous acute or geriatric/adolescent or rehabilitation experience preferred.
Knowledge of growth and development across the life span.
Responsibilities:
The duties outlined here are not exhaustive. The employee in this position is expected to perform these and any additional tasks as assigned by their supervisor or facility Administrator, even if not specifically listed. This role involves comprehensive oversight of resident care, regulatory compliance, team supervision, and collaboration with interdisciplinary teams. Key responsibilities include:
Essential:
Patient Care and Assessment : Conducts patient assessments, collaborates with Registered Nurses (RNs) on care needs, and develops individualized nursing care plans with measurable goals. Engages patients and families in care plans and identifies discharge needs.
Knowledge of Disabilities : Understands the impact of disabilities across physical, emotional, and social aspects, considering age-related and developmental factors (adolescents through geriatric populations).
Direct Care Delivery : Provides patient care under RN supervision, manages patient health status, recognizes changes and emergencies, follows Whittier Rehabilitation standards, and performs basic activities of daily living (ADL) care. Transcribes physician orders and administers medications per policy.
Documentation : Accurately documents patient information with an understanding of legal implications.
Communication : Demonstrates effective verbal, written, and therapeutic communication with patients, families, and care teams. Participates in patient care conferences.
Patient Advocacy and Rights : Upholds patient rights, privacy, and dignity, acting as a patient advocate and incorporating families into care decisions.
Team Collaboration : Prioritizes tasks, delegates appropriately, and supports ancillary staff and team members.
General Standards:
Adheres to hospital policies and safety regulations, maintains a respectful and cooperative attitude, communicates effectively, demonstrates reliability, and upholds a positive community image.
Non-Essential :
Supports policy development, and participates in hospital committees.
This role requires professionalism, dependability, and cooperative work with all stakeholders to maintain high-quality resident care and compliance.
