Proficiency in general Neurosurgery, trauma, spine EMR: Epic Support Staff: none Travel Preferences: Air Travel: Okay; Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Credentialing Information: Timeframe: 30 - 120 days Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? NO Emergency temp privs? Yes, 1-2 weeks Requirements: Active Unrestricted CO license or IMLC (LOQ in hand at time of submission) - required Board Certified Neurological Surgery - required Active ACLS, BLS - required Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked) Clean Malpractice/Background - Highly Preferred Willing to accept a maximum of 3 cases Must include malpractice explanation or self inquiry NPDB report.
