Yes LOCUM COVERAGE ASSESSMENT * Reason for Locum need? Vacancy Scheduling Need * Is the Locum Coverage needed short-term (3-6 months) or long-term (6+ months)? Ongoing * Is the Locum Coverage needed for a specific shift type: Call Only * Expected amount of time in Hospital: * Call Response Time: 30 mins * Call Ratio: Solo * Call Back %: 10 * Phone Consults: 7-10 * Admissions: 1-3 * Temp Privileges granted? Yes with clean files and an active license * Start Date: ASAP PROGRAM DETAILS * Patient Volume: 2-6 * Support Staff: RNs 2 * Will they be required to supervise Advance Practice Providers?
