$107k - $157k DOE BENEFITS:

A master’s Degree in these fields is preferred. EXPERIENCE: At least three (3) years of experience as a

manager, director or administrator

in a human and/or health care services operation serving the aging population. Strongly prefer leadership experience at a

PACE

program. KNOWLEDGE: Current concepts and practices of health and social services for the elderly, particularly in assisted living, in-home and community-based services, and the interdisciplinary team model. The managed care environment includes claims management, management of statistical and medical indicators, and managed care utilization trends and implications for the PACE program. Healthcare delivery and financing systems, including Medicaid, Medicare, waiver programs, prospective payment systems, and Health Management Organizations. Identified county healthcare and related networks. Utilization management, quality improvement, managed care health care concepts, marketing, and program development. Budgeting process. Management information systems, including fiscal management issues. JOB SUMMARY: The

Director of PACE & Senior Services

will be responsible for the leadership, development, and administration of the

Healthy Aging and Wellness Program

which involves leading the programs of Program of All-Inclusive Care for the Elderly (PACE), Assisted Living, Adult Day Health, and Congregate Meal Program services. Duties include ensuring the financial viability of all programs and the continued growth of the PACE program. Responsibilities include managing all business, partner, Local, State, and Federal government and community relationships. JOB DUTIES: Oversees the Healthy Aging and Wellness Program. Successfully leads and manages the Centers for Medicare and Medicaid Services (CMS) Program of All-Inclusive Care for the Elderly (PACE), PACE Third Party Administrator (TPA), and PACE Pharmacy Benefit Manager (PBM) in partnership with the Finance Department. Ability to obtain HPMS access and reporting. Initiates and maintains a relationship with PACE, Assisted Living and Adult Day Health State Agency Administrators, National Pace Association, national, state, and local partners. Initiates and maintains a relationship with CMS Regional office. Oversees PACE Technical Consultant through the PACE application process. Collaborates with PACE Technical Consultant to develop operational workflow. Oversees operational policy and procedure development and management, such as infection control and emergency preparedness. Recommends and implements the Healthy Aging and Wellness Program operational policies, procedures, and protocols for direct participant care and support services. Initiates and oversees procurement of CLIA (Clinical Laboratory Improvement Amendments) license and other necessary licenses to operate Assisted Living, Adult Day Care/Health, Congregate Meal Program and PACE. Develops PACE provider and vendor network. Collaborates with marketing to develop materials and presentations. Represents PACE on the Utilization Management/Resource Management Committee; assists in managing and leading the committee when the COO is unavailable. Collaborates with Human Resources for hiring and performance of staff. Develop equipment list (Clinic, OT, PT, Day Center, Recreation, Furniture) and set up inventory control system – purchase all necessary equipment before operations. Develops and provides orientation, onboarding, and training for staff and contractors based on CMS guidelines and regulations. Oversees tenant improvement and ensures readiness of facility for PACE Readiness Review. Directs, supervises, and evaluates the performance of the Staff Accountant, Medical Director, Marketing Director, Homecare Coordinator, Transportation Coordinator, Quality Assurance Director, Center Director, Executive Assistant, and Human Resources Manager. Ensures that job responsibilities, authorities, and accountability of all direct reports are defined and understood. Maintains compliance with all State and Federal program requirements, obtains and maintains certification under CMS as well as other State or Federal laws or rules pertaining to PACE, Assisted Living, Adult Day Health, and Congregate Meal Program in collaboration with the Medical Director, Center Manager, and Quality Assurance Manager. Monitors medical and specialty services utilization to ensure the provision of quality care to participants in collaboration with the Medical Director, Center Manager, and Quality Assurance Manager. Oversees the financial accountability of the Healthy Aging and Wellness Program and develops and monitors the Healthy Aging and Wellness Program’s annual operating and capital budgets and projected financial benchmarks. Plans for and administers the managerial, operational, fiscal, and reporting components of the Healthy Aging and Wellness Program. Monitors the growth of care delivery and services as the participant census grows and makes adjustments in planning as needed to maintain established growth and service goals. Meets program goals for target enrollment. Oversees and participates in the quality assurance/quality improvement committees as part of the overall quality improvement program. Establishes positive working relationships with providers and the community at large. Oversees operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures, ensuring a safe and clean environment for staff and program participants. Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications. Provides evening and weekend on-call administrative coverage on a rotational basis.
