Summary:
Under general supervision, counsels and aids at-risk individuals and/or families requiring one or more forms of psychosocial services; interviews clients experiencing problems with personal and family adjustments, finances, employment, food, housing, physical and mental impairments, and/or similar areas; secures and evaluates applicable related information; refers clients to community resources and other organizations. The Social Worker is on-site and available seven days a week, as well as holidays and, therefore, is required to work a weekend rotation and an occasional holiday and will be on call as require by departmental scheduling. Provides oversight and direction to the BSW staff.
POSITION EDUCATION/ QUALIFICATIONS:
- Master Degree from an accredited school for Social Work
- Valid Licensed Master Social Worker license to practice in the State of Texas
- Working knowledge of theory & practices of social work is required.
• Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices.
• Provides counseling to patients and families within scope of license.
• Counsels patients and families in crisis.
• Counsels patients and families involved with abuse and neglect.
• Counsels patients and families about end of life issues and planning.
• Responds to referrals from other health team members regarding patient and/or family issues.
• Documents acknowledgement of referral as soon as possible after notification of need for MSW and completes medical record documentation as soon as possible or by end of shift.
• Documentation within the medical record is clear, concise and addresses all issued completely
• Provides leadership, education, and mentoring of Bachelor of Social Work Staff.
• Provides training within the department on topics relevant to the services provided.
• Provides training program of BSW as required by the State of Texas.
• Participates in the hiring and evaluations of the Bachelors prepared staff.
• Maintains personal competencies needed for licensure and attending internal and external educational offerings.
• Evaluations are completed timely and provide goal setting for the future year.
• Documentation of in-services and state required competency assessments for BSW licensure is current for each staff member, (specific medical record review of documentation and actions to address issues identified is completed.
• Assess the psychosocial needs of the patient and family to develop, communicate, and implement an effective and comprehensive plan for psychosocial interventions and complex discharges.
• Functions as a resource to the care team regarding Social Work patient assessment and plan of care development in a patient focused manner.
• Utilizes appropriate resources in cases that present ethical dilemmas
• Will act resource regarding social systems, institutions, and behavioral and relationship patterns
• Skillfully apply the appropriate interventions to meet the needs of the patient and family
• Consistently demonstrates ability to exercise independent judgment and recognize strengths and abilities.
• Communicates the discharge plan to patients/ patient representatives and pertinent healthcare team members.
• Performs social work assessment of patient and support systems in order to facilitate the most appropriate and timely transition plan.
• Assists DHS when there is a need for guardianship
• Participates in Health Care Team Meetings and elicits recommendations from team members regarding how needs may be met.
• Makes appropriate referrals to community resources for social work and discharge planning
• Implements and documents appropriate social/discharge planning services to meet patient/family/significant other needs
• Completes functions for social work to meet needs for financial assistance, health care needs, and placement
• Documents referrals to nursing homes, rehab, hospitals, and home care
• Documents meetings with family, patient, or doctor
• Assesses patients for discharge planning.
• Works closely with victims of abuse and/or neglect, makes appropriate referrals to the Department of Human Services (DHS) and provides appropriate resources
• Assesses all patients referred for possible abuse or neglect according to established policy and works with the Department of Human Resources to provide community resources for shelter, safe discharge plans and counseling as appropriate
• Assesses all infant adoptions/placements for completion of adoption processes/paperwork required according to established policy
• Assists with referrals to appropriate local/state/community resources.
• Responds to referrals within 24 hours of request for SW services
• Documentation within the medical record is completed by the end of the shift and is complete 95% of the time.
• Recognizes and immediately intervenes in cases of suspected abuse or neglect.
• Collaborate with the multidisciplinary team to assure effective, efficient, appropriate care in order to optimize the health status and assure a smooth transition between levels of care for patients served.
• Manages each patient's transition through the system and transfers accountability to the appropriate person upon entry into another clinical service or upon discharge articipates in daily rounds on nursing units.
• Serves as a resource to associates/physicians and Case Managers on issues relating to social services and discharge planning
• Assists in facilitating and implementing physician orders to aid in the timely completion of patient care along the continuum.
• Provide education and expertise to promote the practice and profession of social work within the hospital.
• May be called upon to undertake supervision of students, trainees or staff other than Social Workers.
• In accordance with departmental policy, will be required to attend relevant training courses and staff developmental programs when these are made available and recommended by senior staff/training officers.
• Serve on specialty areas within the hospital that require a SW for regulatory or grant funding requirements.
• Provides training within the department at least once per year on a topic that is applicable to the services provided.
• Collaborates with Quality Management Department: Performs quality assessment reviews and studies both concurrently and retrospectively as required by the hospital's PI plan, JCAHO standards, CMS and third-party payer regulations.
•• Applies generic quality screens/indicators concurrently to patient medical records and accurately abstracts relevant patient care data to determine if quality screens are flagged. Performs first line reviews on potential quality issues as requested by the Director.
• Refers all other potential quality of care issues identified, not reviewed, as part of the quality assessment screening to the physician advisor to facilitate timely follow up.
• Collects potentially avoidable day data for system Performance Improvement.
• Refers quality issues to the Case Management Supervisor, CMO and/or PA appropriately.
• Identifies and reports opportunities to improve care for patients, including patient complaints and any unusual patient care issues.
• Performs related duties as required
Other information:
JOB KNOWLEDGE/EXPERIENCE:
- Three to five (3-5) years' experience is required,
- Excellent communication skills (oral and written) required.
- Must demonstrate an understanding of community health, welfare, and social agencies.
- Working knowledge of Medicare, Medicaid and third party payers preferred.
- Hospital or acute care setting experience is strongly preferred.
