Job Overview:
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our team. In this role, you will coordinate and oversee patient care, ensuring efficient utilization of resources while maintaining the highest standards of care. As an RN Case Manager, you will collaborate with interdisciplinary teams to develop and implement individualized care plans, support patients throughout their healthcare journey, and advocate for optimal health outcomes.
Responsibilities:
  • Assess patients' healthcare needs to develop, implement, and evaluate individualized care plans.
  • Coordinate with healthcare providers, patients, and families to ensure seamless transitions between care settings.
  • Monitor patient progress, assess outcomes, and adjust care plans as needed.
  • Serve as a liaison between patients, families, and the healthcare team to address concerns and facilitate communication.
  • Educate patients and families on treatment options, disease management, and available community resources.
  • Collaborate with insurance companies and other third-party payers to secure necessary approvals and resolve billing issues.
  • Conduct regular follow-ups to ensure adherence to care plans and address barriers to care.
  • Advocate for patient needs while balancing healthcare efficiency and cost-effectiveness.
  • Document all case management activities accurately and in compliance with regulatory standards.
  • Participate in quality improvement initiatives to enhance patient outcomes and operational efficiency.
Qualifications:
Required:
  • Current and unrestricted Registered Nurse (RN) license in AZ.
  • Associate or Bachelor’s degree in Nursing.
  • Minimum of 2 years of clinical experience.
  • Knowledge of healthcare reimbursement systems and regulatory requirements.
  • Strong communication, critical thinking, and organizational skills.
Preferred:
  • Certified Case Manager (CCM) or willingness to obtain within 1 year of hire.
  • Experience with Electronic Medical Records (EMR) systems.
  • Familiarity with community resources and social services.
