Job Responsibilities:- Develop and implement individualized care plans
- Coordinate and oversee the care of patients
- Provide hands-on nursing care as needed
- Collaborate with interdisciplinary teams to ensure the best possible outcomes
- Act as an advocate for patients and their families
- Assess patient needs and provide appropriate interventions
- Active RN license in the state of Georgia
- Minimum of 2 years of nursing experience
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of hospice and palliative care preferred
