1. Promotes a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c)

2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to. Absenteeism, E-Time, cellular telephone usage, dress code and rules of conduct.

3. Make Customer Services a priority, treating customers( patients, visitors and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and other about SBH services in their area(s) of expertise or responsibility and ensure that that concerns, questions or issues reach the right person(s)

4. in SBH in a timely fashion.

5. Interview patient and/or family to obtain psychosocial information and assess the patient's functioning relative to safety and/or discharge planning needs.

6. Formulate an assessment and treatment/discharge plan that will highlight a clinically realistic, and sensitive understanding of the patient's needs.

7. Participate in scheduled multidisciplinary rounds/meetings.

8. Provide direct social work/discharge planning services including community resource advocacy as well as keep ongoing professional rapport with community agencies as needed.

9. Document all work activity in the patient's medical chart and Allscripts accordance with departmental and regulatory guidelines.

10. Engage with patient and/or family members with respect to cultural differences and sensitivity to individuality.

11. Demonstrate consistent attention towards patient confidentiality.

12. Counsel patients, families, and significant others in the areas of expectation regarding life style changes and functional coping patterns.

13. Complete and turn in monthly statistics timely.

14. Attends all scheduled Social Work/Discharge Planning Department meetings.

15. Speaks to patients, families and significant others with age appropriate conversation and demeanor.

16. Provides social work rotating weekend coverage in Pediatric Emergency Department, Nursery, Labor and Delivery and other areas, as necessary.

17. Attend scheduled meetings participate in White Board Rounds, discharge planning meetings and patient conferences, etc. as needed. Provides supporting documentation of activities in AllScripts and Weekend Log

18. Attend scheduled supervision and obtain consultation when needed.

19. All other duties as assigned.

  1. Education Required: Master's Degree in Social Work from an accredited school of social work. Participation in continuing education activities is expected.
  1. License(s) Required: New York State Certification (LMSW). [Employee must either: 1) Possess at time of hire or 2) obtain within 6 months of start date.]
  1. Experience/Skills Required: Two (2) years' experience in a hospital setting, and

related to the department to which the worker will be assigned.
