Ensures patient and family centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care.Examples Of Typical Tasks Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families. Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions and implements therapeutic and corrective measures. Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician’s intervention. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
