A health services organization in New York City is currently seeking a new Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW) to join their Assertive Community Treatment (ACT) team at one of their inpatient mental health facilities in Manhattan.
Responsibilities:
The Social Worker (LMSW / LCSW) will be responsible for:Providing the expected number of services to clients each month, majority of which are in the community or patient's homesCompleting required documentation in a timely and high quality mannerWorking collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goalsCoordinating client care through close involvement with hospitals, substance use treatment facilities, etc.Screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitationsParticipating in the development of treatment plansCounseling and educating individuals, groups and families to promote mental, emotional, physical and social functioningProviding client/patient advocacy and crisis interventionDeveloping, coordinating and evaluating services and support systemsQualifications:
1+ year of relevant work experienceActive NYS Licensed Master Social Worker (LMSW) and/or Licensed Clinical Social Worker (LCSW)Training and/or experience in integrated Mental Health and Substance Abuse assessment & treatment, Motivational Interviewing techniques, and Stage-wise Treatment; or,Training and/or experience in Family Assessment & Intervention, Psychosocial-education and/or other Family Support services, including Cognitive Behavioral strategies
