Duties/Responsibilities
Complete the Case Management Initial Assessments for patients
Provide consultation to members of the Care Team
Provide therapeutic intervention with patients and families
Utilize appropriate case management interventions to address current and/or future barriers to care and well-being
Facilitate referral to support services
Initiate the process of developing a discharge plan with patient
Facilitate family and treatment team meetings early in the admission, as clinically indicated.
Participate in supervision and team meetings.
Adhere to National Association of Social Work ethics and scope of service standards for Social Work Case Management practice.

Experience/Knowledge/Skills Required
MSW/LCSW: Experience preferred
LICSW: minimum 2 years of supervised clinical experience required
Ability to build relationships with patients, family members, community collaborators and colleagues
Effective verbal and written communication skills
Treatment and crisis intervention skills
Knowledge of community resources
Education Required
Masters in Social Work (MSW)
Registration/Certification
Licensed Independent Clinical Social Worker (LICSW) - Preferred
