Responsibilities:Provides social work services to patients and families to assist them in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning.Collaborates with Hospital, New York-Presbyterian System, and community-based personnel to ensure efficient service delivery and a high level of customer satisfaction.
Requirements:Licensure in Social Work in New York State Required.Masters in Social Work (MSW) and a Licensed Master Social Work (LMSW).2+ years in acute care medical social work experience, including hospital discharge planningInpatient discharge planning hospital experience.Must have Epic and Allscripts experience.ER experience required
