Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Center (Isaacs Center) are well-established Manhattan community-based agencies with a combined annual operating budget of approximately $63 million that support services to approximately 30,000 New Yorkers. Goddard Riverside and The Isaacs Center are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults throughout Manhattan and New York City. Pathways for Youth is an effort to help young adults reach age 24 prepared to be self-sustaining by providing a primary adult support to coach and assist in life planning and navigation over multiple years.

Purpose of Position:The Social Work Case Manager works creatively and effectively in the community to engage individuals living on the street through outreach by utilizing a harm reduction approach. The position serves as a case manager for adults clinically complex adults experiencing street homelessness, with the goal of placement into transitional and permanent housing. The position works collaboratively across teams and organizations to achieve this goal.

Schedule:Monday through Friday 9:00 am to 5:00 pm

Salary Range:$56,000 - $61,000

Roles, Responsibilities, and Essential Duties

Serve as Case Manager to a small caseload of adults experiencing homelessness, with a goal of placement into transitional and permanent housing. The caseload will be comprised of clinically complex clients

Provide program support regarding clinical assessment of clients and need for hospitalization, coordinate 9.58 as needed

Provide program support for SPOA applications to obtain mental health wraparound services to those that meet criteria

Perform intake, assessment, service planning, advocacy, referral and escort services

Maintain client case records and enter data in computer database using handheld and desktop computers

Provide follow-up case management for clients placed in housing, to promote retention and document outcomes

Attend staff meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement

Interface with various community agencies, funders and stakeholders relevant to our client population

Participate in outreach trips to identify and communicate with homeless clients

Drive outreach van and public transportation when necessary

Participate in at least four (4) overnight quarterly counts of homeless individuals living on the streets of Manhattan, including the HOPE count

Work with Med Liaison on increasing participation of clients for psychiatric care

Other duties as required

Qualifications/Educational Requirements

MSW degree required within 6 months of hire, LMSW required within 9 months of hire

Understanding of housing first and harm reduction models preferred

Valid driver's license and a driving record that meets the agency's auto insurance guidelines (including no suspensions within the last 3 years)

Proficiency in Spanish/Chinese dialect and English preferred

Computer Skills: To perform this job successfully, an individual should have/be:

Able to work in the Microsoft Office Suite, should especially be proficient in Word and Outlook

Able to use or learn other electronic databases, programs, and funder-related software, such as Mocapp, Streetsmart, HMIS, and AWARDS

Physical Requirements

Frequent walking and standing,

Ability to lift up to 30lbs such as client belongings during moves
