Home Care Social Worker (LCSW) - Per Visit

As a LCSW you will be reporting to the Case Coordinator, you will provide home health services to an assigned caseload; develop the Physician Plan of Care in coordination with the health care professionals assigned to the case; ensure that all home care services are provided in a high quality manner and in accordance with Department of Health regulations; and support the Agency’s mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.Qualifications2+ years of experience in Family Therapy/Counseling; 2+ years of Supervised experience in the Health fieldMaster's in Social Work from a graduate School of Social WorkIndiana Licensed Clinical Social Worker (LCSW)Microsoft Office/Suite proficientPC / IPAD proficientSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedResponsibilitiesContribute to patient and family counseling and educationParticipate in ongoing interdisciplinary comprehensive assessmentAssess patient/family psychosocial response to the illness and evaluate coping skillsAssist patient, family and agency staff in utilizing community resourcesAssist team members in understanding the significant social and emotional factors related to the care of the patient and familyCoordinate with team members to plan and implement the home care plan of careParticipate in interdisciplinary team meetingsMaintain accurate, thorough and professional written records, and charts relevant information in a timely fashionDevelop and implement the discharge planCompensationReimbursement per visit and mileagePRN Position with great flexibility
