As part of the Addiction Consult Service Team, the Community Social Worker will be part of a team of mid-level providers, recovery coaches and behavioral health leaders. The Social Worker will work collaboratively with the existing care team to connect high-utilizing and socially complex patients with the appropriate community resources as required. The Community based Social Worker will work with at-risk patients and families in all settings, including homes. They will assist patients in connecting with any and all available resources with the goal of improving the patient’s quality of life and avoiding unnecessary hospitalizations.
Job Description:
Essential Duties:
Works collaboratively with inpatient multidisciplinary teams as well as Ambulatory Care teams such as Lahey ACO, NEPHO, Eliot, Atrius, etc.
Successfully builds trusting relationships with patients and caregivers
Serves as an advocate for patients and families
Actively connects patients to identified services as requested
Ensure patient has transportation to follow up appointments
Post discharge, the Community Social Worker connects patient to financial or social services such as food, shelter and transportation as needed
Additional connections include family support, support groups such as AA/Grief/NA
Communication with patients either by phone, text or face to face
Assists patients in connecting to detox or behavioral health services
Works collaboratively with homecare services post discharge
Build relationships with community resources such as behavioral health organizations, elder care services, jail diversion programs, substance abuse programs, housing authority, community shelters and other organizations that may benefit patients
Knowledge, Skills & Abilities:
Strong networks with Essex County community resources and the ability to create new relationships
Ability to work collaboratively
High energy and a sense of humor
Ability to overcome obstacles in a positive, productive manner; problem solving skills are critical
Strong communication skills, both written and oral
Strong documentation skills
Possess a working knowledge of Section 35 Petitions, Section 12 Petitions, and protective care issues with general knowledge of the social work judicial system
Hospital inpatient and psychiatric inpatient experience a plus
Required Education/Experience:
Masters in Social Work or related field, with license
2 or more years of direct patient care
Willingness and ability to perform outreach services
Must have knowledge of medical and medication terminology
Working knowledge of health care plans and benefits
Familiarity with PC’s and MS Office software applications
Own transportation required.
