Job Description:The Care Management Social Worker provides social work services to patients and families, including assessing needs, formulating treatment plans, and coordinating care with the interdisciplinary team. They offer counseling, crisis intervention, and advocacy services, ensuring patients receive comprehensive care and support. The role involves collaborating with community agencies, leading educational groups, and maintaining documentation standards. The ideal candidate holds a Master of Social Work Degree, is a licensed social worker (LMSW/LCSW) in New York, and has at least two years of experience in an acute care hospital setting or related environment. NYS SCREEN Certification is required within the initial 3 months of employment.

Responsibilities:

Conduct interviews with patients and families to determine continuing care needsComplete Social Work Comprehensive AssessmentsProvide psychosocial information for patient transfersOffer counseling, crisis intervention, and grief counselingAssist patients with adjustment to treatmentDevise and implement social work treatment plansProvide information, referral, and advocacy to community agenciesLead education and support groupsIdentify, evaluate, and design safety plans for victims of crimes/abuseMaintain and complete statistics of case activitiesAttend meetings and participate in presentations, training, and in-services as requiredAssist/participate in presentations, training, and in-services as requiredParticipate in the department's cross-training programRespond to internal and external disasters when indicated/assigned by supervisorPerform other duties as assignedRequirements:

Master of Social Work Degree from a Council of Social Work Education accredited School of Social WorkLicensed Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New YorkTwo years experience in an acute care hospital setting or related settingNYS SCREEN Certification (obtained within initial 3 months of employment)Ability to relate cooperatively and constructively with patients, families, significant others, co-workers, physicians, and other staff membersPhysical/Mental Demands/Requirements & Work Environment:

May be exposed to chemicals necessary to perform required tasksMust be physically able to write, read, and move quickly throughout the work environment and hospitalMust have the dexterity to operate computers and other office equipmentMust have the ability to assess a patient's situation, formulate a plan according to need, and follow through to resolutionMust be able to communicate with patients and others in a sensitive and caring manner and to visually assess a patient's affect and body language
