Essential Functions:
Perform the following duties for assigned caseload (in multiple service types)
Maintain case records in accordance with licensing and Medicaid standardsCrisis intervention and/or case management servicesCoordinate, develop, and implement Individualized Service PlansConduct team meeting to incorporate all valid input into the Individual Service Plan developmentProvide trauma informed, person centered, psychosocial and psycho-educational skill building interventions with each client to achieve treatment goalsProvide individual, family, and group counseling and therapy in keeping with approved credentialing and skill setFacilitate individual and family sessions to address identified goalsDocument all treatment activities to include phone callsOngoing and consistent communication with affiliated case professionals to include school, criminal/juvenile justice, social services, MCO/Payor, and other mental health providers weeklyProvide formal case updates and presentations to professionals in various settings (i.e., FAPT, IEP, Court)Employment benefits include:
