Responsibilities Include:
Monitor patients' medical treatment and ensure follow-up care.Facilitate communication between patients and healthcare providers.Reach out to patients due for preventative care measures.Enroll patients in various medical programs.Engage in consultations with patients and their families to address health concerns.Provide comprehensive education to patients about their medical conditions, medications, and care protocols.Maintain accurate patient records and documentation.Coordinate care and support for patients with multiple healthcare needs.Assist patients in navigating healthcare systems and accessing services.Ideal Qualifications:
Experience in customer service.Strong computer skills.Ability to handle multiple tasks concurrently.Experience in care coordination or a related field.Excellent communication and interpersonal skills.Compassionate and patient-focused approach.Strong organizational and multitasking abilities.
