A healthcare network in New York City is is currently seeking a Licensed Master Social Worker (LMSW) and/or Licensed Mental Health Counselor (LMHC) to join their growing team in Brooklyn.
About the Opportunity:Schedule:
Monday to FridayHours:
8:30am to 5pm (2 late days a week 10:30 to 7pm)Patient Population:
Ages 55+ (asylum seekers, undocumented, etc.)Caseload:
55 clientsResponsibilities:
The Social Worker (LMSW / LMHC) will:
Complete admission screening and conduct psychosocial assessmentsMake treatment recommendations and participate in treatment planningProvide social casework services; provide individual, group, and family therapyDevelop and implement discharge plans and and coordinate services with community providersReview the individuals psychiatric, medical and other case records and discuss issues with families and other members of the treatment teamEstablish and maintain a therapeutic relationship with individuals and their familiesQualifications:
Bachelor's DegreeValid NYS Licensed Master Social Worker (LMSW) and/or Licensed Mental Health Counselor (LMHC)Your privacy is important to us.
