A healthcare organization in New York City is currently seeking a Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) to join their Hospice program in Queens. In this role, the Hospice Social Worker will will be responsible for providing psychosocial support counseling and other social work related services to palliative care patients and their families in both community homecare and facility settings.

Serve as a member of the Palliative Care interdisciplinary teamProvide comprehensive psychosocial report to the teamDevelop the palliative social work plans of care and the goals associated with those plansMake recommendations based on patient/family needs for an active caseloadRefer patients to Hospice whenever appropriatePerform other duties, as neededQualifications:
Bachelor's DegreeNYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)Desired Skills:
Master's DegreeHospice and/or Home Care background
