Job Description

Purpose :

Under professional supervision and as a member of an interdisciplinary team, provides clinical social work services individual, group, and family to adult and geriatric psychiatric patients registered in the Assertive Community Treatment Team.

Specific Duties and Responsibilities:

The Social Worker III is charged with and responsible for:

Provides clinical supervision, prepares evaluations and competencies for assigned social service staff, and/or graduate students.Orients and trains new employees.May have in charge duties, in the absence of Program Director, as requested.Prepares and presents education material for social work and other professionals.May lead or work on various committees/task groups for the department.As primary therapist and case manager, is responsible for psychosocial assessment, treatment planning, discharge planning and direct care to patients on caseload.Provides individual and family psychotherapy, intakes psycho-education, substance abuse counseling, psychiatric rehabilitation and crisis intervention services.Coordinates psychiatric and medical treatment, as required. Provides concrete social services assessment and interventions.Makes home visits and escorts patients to various appointments, as needed.Provides on call, beeper coverage on a rotation basis.Participates in conferences, training programs, performance assessment and improvement activities, departmental, discipline and team meetings. Prepares and presents in-service education.Prepares and maintains appropriate records, summaries and reports following the guidelines of assigned unit.

Minimum Qualifications

For Assignment to Level III1. Valid license as an LCSW issued by the NYSED.
