Job Description

TOUR 2 (9am-5pm)

PUPROSE OF POSITION:

To ensure that all eligible patients receive HRA 2010E packages for housing placement. To ensure social work staff under area of responsibility are carrying out all job responsibilities appropriately.

AREAS OF RESPONSIBILITIES

:

Social Work Staff located at the five CHAT/HRA sites

SUMMARY OF DUTIES AND RESPONSIBILITIES:• Prepares psychosocial evaluations of clients.• Will assess clients off site as necessary.• Has ability to work remotely at least two days per week.• Prepares HRA 2010E housing applications for all eligible clients.• Provides concrete services and referrals as necessary.• Maintains statistical data regarding interventions using Microsoft Excel and Microsoft Access Programs.• Documents all interventions and activities in the patient's electronic record in accordance with departmental policies.• Attends necessary Social Work and Comprehensive Health Assessment Team meetings• Develop schedules and programmatic time tables for the functioning of all five sites.• Assists in the preparation of budget estimates and controls to ensure that expenses are contained within the budget.• Assists in the development of staff role and responsibilities.• Assists in the enhancement of staff clinical skills.• Responsible for the clinical and administrative supervision of staff.• Responsible for staff evaluation and development.• Serves as a member of the interdisciplinary team to assess housing needs of persons with HIV related illnesses.• Liaise with HRA/HASA/HHC Administrators in matters pertinent to CHAT• Performs other related functions as determined by supervisor.• Reports to Social Worker Level V.

Minimum Qualifications

For Assignment to Level III1. Valid license as an LCSW issued by the NYSED.
