Job Description
9:00 A.M - 5:00 P.M
SUMMARY DUTIES AND RESPONSIBILITIES
Review the medical records of patients in order to identify those whom will require SUD intervention and counseling
.Interview patients to assess their insight into their addiction and readiness to accept treatment.Initiates patient referrals to appropriate in-house or community-based SUD programs.Completes Social Work Assessment for these high/moderate-risk patients as needed.Maintains effective communication and provides supportive counseling, and crisis intervention to the patient with regards to addiction treatment options and services
.Collaborates as a liaison with Chemical Dependency department staff/programs
.Documents all Social Work interventions in the patient's electronic medical record in accordance with departmental and hospital policies.Maintains effective communication with the interdisciplinary team and participates actively of interdisciplinary discharge planning rounds and records plans/recommendations in the patient's medical record.Provides shadowing for new ED Leads staff and coverage to CATCH when designated. Acts as a resource for SW staff.Assist in the orientation and training of new staff of the SW department and other disciplines as needed.Will provide individual and group therapy to patients receiving services through the Chemical dependency department as needed.Complies with requirements for supervision, training, Performance improvement projects, Quality Assurance and Research Studies.Understands and follows the escalation process as needed.Minimum Qualifications
For Assignment to Level II1. Valid license as an LMSW issued by the NYSED; or2. Valid license as an LCSW issued by the NYSED.
