Job Description

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full time (35 hour work week) Licensed Social Worker to support our Inwood Ave site in the Bronx, NY

POSITION SUMMARY

The Social Worker provides specialized knowledge and skills for psychiatric and mental health evaluation, assessment and management of clients. Strong therapy skills are essential. The position reports to the Associate Vice President of Substance Use Disorders.

ESSENTIAL FUNCTIONSProvide psychotherapy for patients.Provide psychosocial evaluations for patients.Practice in collaboration with psych NPs, psychiatrist and other medical staff to manage patient care.Assess the multidimensional needs of patients and develops and implements care plans.Function as a patient advocate, by participating in clinical meetings, patient care conferences and informal discussion with health care and mental health program staff.Provide patient education relative to psychiatric mental health issues.Provide written reports and care coordination as needed.Attend and participate in continuing educational programs, seminars, etc., designated to maintain professional competencePerform all related duties, as assigned, or unrelated duties, as requested, during emergencies.Demonstrate an understanding of the management and care of the adult substance abuse population and patients living with HIV.

EDUCATION/EXPERIENCE:

Master's Degree in Social Work is required

2+ years' experience in the psychiatric mental health care of persons with HIV/AIDS and SUD

Bilingual English/Spanish preferred

Active NYS Licensure (LMSW or LCSW)

Salary Range: $65,000 - $85,000 based on licensure and years of experience.
