Job Description

PURPOSE OF POSITION:

Under supervision, provides social work services, complete psychosocial assessments, coordination of care planning, utilizing a variety of techniques such as forensic interviews, individual, family, group work, and community networking to patients serviced in HIV services.

DUTIES & RESPONSIBILITIES:• Conducts interviews with patients and family member(s)/significant others(s) to determine care needs.• Completes Social Work Comprehensive Assessment for all patients referred or in need of social services in accordance with departmental high/moderate risk criteria policy.• Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/ significant others as appropriate.• Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/ or anger; noncompliance with medical regimen.• Devises/implements a social work treatment/continuing care plan involving the patient, family/significant other, and the interdisciplinary team.• Documents on the patient's medical record in accordance to the social work departmental policy.• Provides information, referral and advocacy to community agencies.• Advocates and serves as a liaison for patients with community agencies.• Coordinates individual interdisciplinary case conference as needed.• Leads education and support groups for patients and/or families/significant others as appropriate.• Identifies, evaluates, provides supportive counseling and refers victims of crimes/abuse, i.e: child abuse, elder abuse and domestic violence to appropriate resources.• Attends individual, unit, departmental and intra/inter hospital meeting as mandated by the supervisor and/or department director.• Assists /Participates in presentations, training and in-services as required.• Participates in the department's cross-training program.• May supervise social work staff and students• Responds and participates in external and internal Disasters when indicated/assigned by supervisor.• Performs other duties assigned by supervisor as needed.• Complies with all Bronx Family Adolescent Child Consortium (BFACC) program standards.

Minimum Qualifications

For Assignment to Level II1. Valid license as an LMSW issued by the NYSED; or2. Valid license as an LCSW issued by the NYSED.
