Job Summary:
The Department of Obstetrics and Gynecology at SUNY Downstate Health Sciences University is seeking a part-time Community Outreach Coordinator. The successful candidate will:
Manage programmatic partners and establishes referral pathways.Develop a portfolio of 15 to 25 organizations and cultivates key relationships at all levels of staff as well as with clients.Achieve visit targets set by management.Coordinate outreach events and programs.Build relationships with internal programs and services and act as a liaison between external referral sources and internal programs.Track outreach activities in agency Salesforce CRM or tracking tool.Create and implement new marketing strategies and plans to achieve targets.Participate in Community Engagement team meetings and participate in team huddles.Complete required documentation.Assist client with linkage to intake and assessment services.Organize health home intake assessment days at community partner sites.Participate in presentations to advance relationships with key target audiences.Locate and participate in Health Fairs, Conferences, and other events to increase awareness and visibility. Evening and weekend work may be required.Perform other duties as assigned by Supervisor.Required Qualifications:
High School Diploma or equivalent.2+ years of applicable experience and/or equivalent experience.Proficient in Microsoft Office.Excellent communicator with a willingness to learn and collaborate with others.Strong work ethic.Preferred Qualifications:
Experience with virtual outreach and graphic design a plus.

Variable Days; Variable Hours
