Summary:To promote the mission to help individuals achieve a more independent and fulfilling life by being an effective team member.Responsible for the delivery of clinical services, to adolescents and their families who are either at risk for or actively engaging in substance use/abuse.
GENERAL RESPONSIBLITIES:Understanding PATH's mission and how your job is essential to meet that missionBeing an effective and engaged team memberBe knowledgeable of your job descriptionUnderstand the expectations of your jobTreat everyone with respect recognizing individual and cultural differencesBe attentive to program goals that tie into PATH's mission and ensure that you do your part in achieving these goalsAccept coaching and supervision in a positive and constructive mannerListen and accept feedback from your supervisorDiscuss your expectations and performance with your supervisor to ensure your successful performanceParticipate in your Performance EvaluationUnderstand that failure by you to perform your duties will negatively and adversely affect service provision and your other team membersCommunicate with your supervisor on an ongoing basisMaintain an active dialogue with your supervisorActively participate in individual supervision/team meetings/department meetingsProvide timely feedback as to the status of the work you are doingProvide input and raise questionsEnsure your knowledge of agency and program policies and proceduresRemain open and understand the rationale for decisions made. Follow through on decisions.Complete task assignments within required time frameMaintain high morale and cooperate with your team and with other departmentsSupport a culture of trust, integrity and confidence by:Conducting yourself in an ethical and professional manner in working with individuals whom we serve, families, the community at large, co-workers, colleagues from other organizations, funding sources, etc., through:Knowing PATH's Code of Conduct and adhering to itRespecting the confidentiality of the individuals whom we serve and ensuring you are knowledgeable and adhere to PATH's policies and procedures regarding confidentialityMaintain all credentialing standards and requirements for your position and continually enhance your own knowledge and expertise and share this effectively with your teamFollow through on commitments you have madeAccept appropriate delegationEnsure that quality services are continually provided:Identify programmatic issues, develop recommendations and present them to your supervisorKeep your supervisor informed of progress and issuesAdhere to your schedule to assure appropriate coverage at all timesRespond in a timely manner to program/departmental emergencies and unusual circumstances and inform your supervisor as soon as possibleBe conscientious and exhibit cost consciousnessWork cooperatively with other departments within PATHRepresent PATH and your program at both internal and external meetings, as requestedSubmit leave requests in accordance with department procedures to assure that the needs of the department can be balanced with individual requestsComplete timekeeping per agency proceduresAttend all mandated trainings within required timeframe and request approval for staff development opportunities as relevant to promote growth and skill buildingPerforming all other duties as assigned
SPECIFIC DUTIES:Provide individual, family, couple and group therapy for adolescents and their families utilizing approved treatment modalities and interventionsProvide case management services for assigned youthBe aware of clients' financial liabilities, coordinate any financial changes with PATH's Billing Department, and assist clients in securing third party resources when indicatedComplete all required documentation including treatment plans, assessments, annual reviews, progress notes and discharge summaries in a timely, comprehensive and legible manner and in accordance with PATH policies and proceduresContribute to any quality improvement/outcome measurement tracking, development and data gathering as assignedCollect all pertinent information and provide comprehensive assessments in accordance with PATH policies and proceduresBe flexible in scheduling appointments to meet youth/family needsMeet productivity standards for your positionWork 2 - 3 evenings per week.
Skills:PA State LicenseExperience working with youth/ familiesMaster's degree in human services field from an accredited college or universityKnowledge of substance use issues and treatment; excellent interpersonal skillsBasic Computer skillsConsistently display a positive attitudeExercise sound judgmentDemonstrate effective communication skillsMaintain a professional demeanorDemonstrate integrityTake initiativeDemonstrate positive interpersonal skillsUtilize effective problem solving skills
