ESSENTIAL FUNCTIONS

Completes assessments, intake evaluations, outpatient assessments, person-centered treatment plans, family/individual therapy (in various settings office and in field)Assists in program development and implementationProvide the professional expertise necessary to make determinations regarding patient eligibility.Make recommendations related to the care needs regarding the clients' health and safety.Participate in child and adult case coordination meetings and assist participants as needed with accessing resources, as well as obtaining consent and authorization formsIdentify and respond to high-risk participant encounters with elevated safety concerns, following company protocolsScreen & Access for acuity levels. Determine appropriate programs based on client needs & risk assessments.Participate in regularly scheduled meetings with supervisors, monthly team meetings, and teaming meeting meetings with collaborativeMaintain the highest standards of confidentiality as described in HIPAA Privacy & Security policies and ensure compliance with annual training requirementsComplete paneling to all insurances needed for Hillcrest consumersREQUIRED EDUCATION AND EXPERIENCE

Master's degree in social work, Psychology or related discipline is required.Must have one of the following District of Columbia certifications: LICSW, LPC, LGPC or LGSW
