Description

POSITION SUMMARY

Transportation of eligible clients to and from Polara treatment facilities or other community providers and various other duties as related to the conveyance of materials and the scheduling of fleet maintenance. Maintains accurate records for billing and vehicle utilization or maintenance. Coordinates all pertinent issues surrounding transportation with the appropriate Staff to include the Access Center. Predominantly in a Polara Health vehicle with clients, driving 5-6 hours per day in city and highway traffic. Some office work and walking between facilities for delivery of agency mail.

ESSENTIAL RESPONSIBILITIES

Transport eligible clients to and from authorized services.Provide courier service and mail delivery between Polara Health facilities.Maintain required paperwork in accordance with agency standards.Report agency vehicle problems to appropriate staff as necessary.Assist in scheduling fleet maintenance.Perform other duties as assigned.Qualifications

QUALIFICATIONS

21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positionsMust be able to provide or obtain an AZDPS Level One Fingerprint Clearance CardMust possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organizationMinimum of a High School Diploma or GED required1 year of transportation experience preferredPHYSICAL DEMANDS & WORKING CONDITIONS
