Hospice of the Piedmont is currently seeking a

full-time Social Worker

to join our caring and compassionate team to provide direct casework and counseling services to patients and families.

• Provides psychosocial assessments, direct casework, and counseling services to patients and families

• Participates in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished

• Works collaboratively as a member of the Interdisciplinary Team and participates in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested

• Prepares documentation for the clinical record per Hospice standards and ensures all documentation accurately represents all services

• Demonstrates understanding and adherence to NASW Code of Ethics and related organizational policies, including confidentiality regarding patients, families, staff, and the organization

• Maintains and updates knowledge of community resources. Facilitates referral of patients/families to these resources as needed and desired

• Participates in evening/weekend coverage responsibilities

• Maintains quality and productivity standards

• Applies knowledge of emotional, social, and spiritual needs and responses to death and grief including factors that indicate complicated grief and high survivor risk

• Facilitates bereavement support groups as needed

• Participates in providing support to staff and volunteers in coping with job-related stress

• Participates in agency and community programs, including supervision, to enhance learning and performance and remains current in topics/research in hospice and grief.

• Participates in the orientation of volunteers and staff and supervises field placement students when applicable

• Facilitates communication with outside agencies including Region 10 and Social Services.

• Implements and plans for placement and discharges from facilities, hospital, and hospice as well as level of care changes

• Takes primary responsibility within the IDT for Advance Directives, Bereavement referrals, APS referrals, funeral planning, volunteer referrals, correct demographics in the EMR, current insurance information and paperwork related to discharges and level of care changes and placements

• As applicable, provides MSW supervision to Bachelor's degree Clinical Social Workers

• Completes other assignments and tasks as requested Typical Physical Demands/Working Conditions: o Sit, stand, stoop, bend, reach, lift and walk, climb steps in patient homes, office settings and facilities o Communicate by phone, writing speaking o Lifting up to 20 pounds o Drive short/long distances o General fast-paced work environment

Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs

Must be able to complete EMR at bedside in varied environments

Requirements

Duties include but are not limited to, provide psychosocial assessments; apply knowledge of emotional, social and spiritual needs in response to death and grief including factors that indicate complicated grief and high survivor risk; facilitate bereavement support groups; participate in the preparation, evaluation and execution of the interdisciplinary care plan; participate in the interdisciplinary team meetings; prepare documentation for the clinical record; and demonstrate understanding and adherence to NASW Code of Ethics and organizational policies.

MSW or current Virginia license as a LCSW preferred; requires a minimum of a Bachelor's degree in social work, psychology or sociology. Candidates must have previous hospital experience with working knowledge of the process of death, dying and bereavement and an understanding of psychosocial factors operative in terminally ill patients and their families. Must have demonstrated knowledge of principles and practices of social work, counseling techniques, hospice philosophy, accessing services through support agencies and be skilled in counseling, interviewing and communicating.
