PRIMARY

PURPOSE:

Provide intensive in-home treatment and support services to families in which there is a risk for out-of-home placement of children and families in which a child is returning from residential treatment. Implement individualized, strengths-based, culturally competent clinical and support services including on-going assessment, individual and family therapy, treatment planning, case management, skill development, behavior management and system coordination. Utilize a wraparound approach, link clients with formal and informal resources and coordinate services.

PRIMARY DUTIES:

A. Complete thorough psychosocial and family assessments on assigned clients, assessing clients' strengths, needs, abilities and preferences. Complete assessments within the designated time frames.

B. Design individualized treatment plans that reflect the assessed needs and preferences of the child/youth and family, building on their strengths, resources and abilities.

C. Develop an individualized crisis plan for child and family to follow.

D. When clinically indicated, develop and facilitate a family team meeting for each case assigned. Develop and encourage natural supports to attend family team meetings. Ensure that all collateral family contacts are included in the monthly family team meeting.

E. Provide individual and/or family therapy and clinical case management per the treatment plan.

F. Maintain at least weekly face-to-face contact with clients in their homes (unless another plan is developed with direct supervisor). The required amount of face-to-face contact will vary from case to case.

G. Coordinate services outlined in the treatment plan, ensuring that services and interventions are carried out, monitoring progress towards goals and modifying the plan as needed.

H. Link clients with formal and informal supports in their community.

I. Ensure clients receive access to crisis intervention services through Hillside as needed on a 24-hour basis, seven days a week.

J. Be an advocate for the client and family regarding juvenile courts and interaction with social services agencies.

K. Teach, coach, role play and role model skills with children and families.

L. Fully participate in weekly one-on-one sessions with direct supervisor, weekly team meetings, and other meetings as required.

M. Consult as needed with psychiatrist/therapist/agencies, including coordinating and participating in meetings.

N. Document all services in accordance with program policies and procedures.

O. Maintain client records in accordance with Hillside, State, Licensing and accreditation guidelines.

P. Represent the program in a professional manner in all dealings with outside agencies, individuals and referral sources.

Q. Serve as the Community Programs weekly on-call designee, on a rotating basis (backing up primary workers when they are unable to respond). This includes being available to respond to crises by phone and in person (if warranted) 24/7 while on call.

R. Perform other duties as deemed necessary by Management.

Demonstrate timely completion of strengths-based plans that incorporate client input.Through Progress Notes, Treatment Plan updates and case staffings, demonstrate progress toward the goals identified through the assessment and treatment planning process.Be able to demonstrate consistent contact with the client, linkages to community-based supports, and skills taught to the client.Be able to demonstrate that client records are maintained in accordance with policies, procedures and recognized best practices.

Qualifications

EDUCATION AND

EXPERIENCE:

Masters Degree in a Human Services field and two years of direct paid service working therapeutically with children, adolescents and families, including case management post masters degree. Licensed or Licensed Eligible is needed. Training in Dialectical Behavior Therapy (DBT) is desired. Be able to effectively use DBT in the therapeutic treatment of children and their families/caretakers.

PHYSICAL

REQUIREMENTS:

Must submit to an annual physical exam.

MISCELLANEOUS

REQUIREMENTS:

A. Must not have a record of a felony or misdemeanor conviction.

B. Must have a current valid Georgia Driver's License. Must present a MVR within one week of request that meets standards set in Hillside's driving policy. Must maintain automobile insurance liability levels of $100,000/$300,000/$100,000 at all times.

C. Must have reliable vehicle to get to and from daily appointments, and be willing to use this vehicle to transport clients when necessary.

D. Must be able to work a flexible schedule that meets the needs of the families Hillside serves. This includes working evenings and weekends when needed.

E. Maintain vehicle insurance coverage as required by Georgia law, Hillside policy and contracts. NOTE: Any exceptions to the above will be at the discretion of the Chief Executive Officer.

F. Must demonstrate excellent writing skills.

G. Must be a self-starter and have the ability to work independently of close supervision.

H. Must exercise good, sound independent judgment and action.

I. Must be willing to be part of a team and be receptive to supervision.

J. Must complete all training's and attend all meetings required by the agency.

K. Hillside is a drug-free work place.

SUPERVISION

RECEIVED:

Guided by verbal instructions and written policies, procedures and established guidelines along with regular meetings with Program Manager.

WORK

SCHEDULE:

Work hours vary as business needs dictate. Subject to be on-call 24 hours a day for crisis intervention.
