Summary/Objective:

The Home Visitor provides early, continuous, intensive and comprehensive child development and family support services to promote positive outcomes for eligible pregnant women, infants, toddlers and their families. The Home Visitor designs and carries out individualized, developmentally appropriate, and culturally sensitive experiences with families in accordance with Child & Family Division established curriculum guidelines, Office of Head Start program performance standards, and the individual needs of the child & family. This includes conducting weekly home visits (60-90 minutes), facilitating twice-monthly group socializations (Play groups), and build trust to establish and maintain positive, goal-oriented relationships to support families in achieving goals set through the Family Partnership process.

Essential FunctionsComplete weekly (child) or bi-weekly (expectant families) home visits with a caseload of 10-12 assigned families on a year-round basis, and fully utilize the Parents as Teachers (PAT) (and/or another approved curriculum).Designs and maintains lesson plans that is developmentally-appropriate, and nurturing to foster children's social, emotional, physical, and cognitive development, and to successfully engage families in a trusting and caring relationship, including those with diverse learning needs and those for whom English is a second language.Complete enrollment paperwork for families entering the program.Partners with families to support children's learning. Expected practices include: practicing anti-bias education principles, valuing diversity, inviting input, responding to concerns with sensitivity and respect, and sharing strength-based observations of child's & parent's skills, interests, and abilities.Partners with families to develop and maintain Individual Family Partnership Agreements with assigned families that support them in reaching family stability and school readiness goals for themselves and their children.Assist in the development and implementation of outreach and recruitment activities designed to reach eligible families most in need of home visiting services.Provide expectant families with individualized education on fetal development, labor and delivery, postpartum recovery, maternal depression, benefits of breastfeeding and pregnancy risks from smoking and alcohol.Ensure that all expectant families enrolled in the Home-Based program schedules a Postpartum support visit within two weeks of giving birth.Assist expectant families with coordinating prenatal/postpartum appointments, dental visit and other community needs.Incorporates the Pyramid Model for Promoting Social Emotional Competence in Infants and Young Children into routines, interactions, and activities.Interacts respectfully with children, families, and colleagues using strength-based, trauma-informed, and relationship-based practices.Identify and report potential concerns with children and/or families and work collaboratively with content area and/or management staff to address them.Work with content area staff to ensure that required health and developmental screenings are completed within 45-day deadlines.Ensure medical and dental determinations are received within 90-day deadlines.Complete quarterly assessments of assigned children's development using PAT Milestone School Readiness Screening Form and input accurate data in a timely manner.Conducts screenings and regular observations of children in the home environment for the purposes of: completing developmental assessments, individualizing instruction, assessing and reporting on children's progress toward learning objectives, and providing timely referrals for services as needed.Communicates regularly with content area staff to support families during program transitions into center-based programs.Actively participates in reflective supervision, team meetings, training, coaching, reflective practice, continuing education, and other educational activities to promote team building and professional development.Designs and facilitates twice-monthly socializations (play groups) which meet the needs, interests and schedule of enrolled families.Refer families to community resources and follow-up on the effectiveness of referrals.Establishes and maintains a relationship of cooperation and respect with colleagues.Identify and develop community partnerships to provide needed services to families.Maintain accurate, timely, and comprehensive documentation in data management system, regarding but not limit to communication with families, home visits cancellations and reason(s).Works collaboratively with supervisor and Director of Professional Development and Coaching to develop and follow an Individualized Professional Development Plan (IPDP) to guide short and long-term training, educational, and coaching goals and plansMaintains the highest degree of professionalism at all times, including, but not limited to: adherence to dress code, to Child & Family code of conduct, and to consistency of care through regular attendance and punctuality.As a mandated reporter, recognizes and reports suspected physical and sexual abuse to the appropriate authorities as required by law.Maintains confidentiality at all times.Other duties as assigned.Competencies

Excellent interpersonal and communication skills (written & verbal).Knowledge of Early Childhood Development and Family Systems.Proficient computer skills.Bilingual skills: English/Spanish.Takes responsibility for delivery of services as part of team effort in unit/department.Takes initiative in making suggestions for improvement of services.Meets deadlines without causing undue disruption.Utilizes human resources, time, and supplies efficiently.Incorporates organization mission, goals, and policies into daily work to foster an inclusive, supportive, and culturally responsive environment for children and adults.Positively represents the Dimock Center in all interactions with visitors, families, colleagues, and community partners.

Work EnvironmentThis position is primarily out in the field.

Physical Demands

Duties may require standing and walking, or sitting for long periods of time.

May require lifting up to 40 lbs., with frequent carrying of up to 25 lbs.

Position Type/Expected Hours of WorkMonday - Friday 8am - 4pm.

Required Education and Experience

A home-based CDA or comparable credential, or equivalent coursework as part of an associate's or bachelor's degreeAssociate's or Bachelor's Degree in Early Education, Health or Social Services, or related field preffredMinimum of three years of experience working in early childhood education, prenatal care, home visiting, teaching and/or community health. Other combinations of education and experience will be considered
