Job Summary:The Manager for Case Management services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital -based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department.
Qualifications/Requirements:
Associate's/ Bachelor's Degree in Nursing OR Master's Degree in Social Work3+ years' experience in Acute-Care Case Management or Social Work2+ years of supervisory experienceBenefits:
