Licensed Medical Social Workers
for immediate
full-time
opportunities within our
inpatient Medical Surgical
and
Float units . As a member of our inpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understand the impact of their diagnosis, and coordinate complex hospital discharges. Additional duties will include:Collaborating with other professionals to evaluate patients' physical and/or mental condition and to assess their discharge needsActing as an advocate for patients to resolve crises and provide appropriate interventionsInvestigating suspected child abuse or neglect cases and initiating referrals to State Central Registry as necessary.Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan.Referring patients to the community or social service programs to meet their complexRequirements
Master's Degree in Social WorkMUST have New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)Hospital/Medical experience preferred but
new grads are welcome .Knowledge of medical terminologyUnderstanding the roles of the healthcare teamExceptional interpersonal skillsDeep understanding of the psychosocial aspects of illness and healthKnowledge of community-based organizations and resources for discharge planningSuperb written and oral communication skillsStrong crisis intervention and short-term counseling skillsExcellent time management skills and ability to work in a fast-paced work environment.Must be able to work Weekend and Holiday coverage on a rotating basis.
