Job Description

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Licensed Master Social Worker to support our Wyandanch site.
POSITION SUMMARY
The Licensed Master Social Worker provides specialized knowledge and skills for psychiatric and mental health evaluation, assessment and management of clients. Strong therapy skills are essential. The position reports to the Senior Vice President of Behavioral Health
ESSENTIAL FUNCTIONSProvides psychotherapy for patients.Provides psychosocial evaluations for patientsPractices in collaboration with psych NPs, psychiatrist and other medical staff to manage patient careAssesses the multidimensional needs of patients and develops and implements care plansFunctions as a patient advocate, by participating in clinical meetings, patient care conferences and informal discussion with health care and mental health program staffProvides patient education relative to psychiatric mental health issuesProvides written reports and care coordination as needed.Attends and participates in continuing educational programs, seminars, etc., designated to maintain professional competencePerforms all related duties, as assigned, or unrelated duties, as requested, during emergenciesDemonstrates an understanding of the management and care of the adult substance abuse population

EDUCATION/EXPERIENCE:
Master's Degree in Social Work, Mental Health Counseling, Counseling, Psychology, Family Therapy, or related field as recognized by the US Department of Education. At least 2 years' experience in the psychiatric mental health care of persons with HIV/AIDS. Bilingual English/Spanish preferred, but not required for the Brooklyn and Queens sites.
Salary Range: $65,137.80 - $76,895.00 annually
