Job Description
Purpose of Position:
A Social Worker is needed to provide patient care services to JMC/ NCB patients that are admitted into the Inpatient units. To provide the patients with the requirements of going back into the community
Responsibilities:
Write a psychosocial assessments on all new admissions to determine patient healthcare needsFormulate and initiate appropriate discharge planning options. Responsible for all documentation that will facilitate patient's dischargeFacilitate and organize outpatient care with providers if appropriateObserve and identify verbal and nonverbal behaviors. Communicates, listens, and elicit expression to understand patient's family perception of the situation. Identify the elements involved in a medical situation and their relationship to the patientsMinimum Qualifications
For Assignment to Level III1. Valid license as an LCSW issued by the NYSED.
Department Preferences
Experience in dealing with mentally ill patients, knowledge of community agencies
LCSW required
