Pathways to Housing DC (Pathways DC) is based in Washington, DC and serves adults recovering from/experiencing homelessness in the District and Montgomery County, MD. We are an innovative and nationally recognized nonprofit committed to ending homelessness for individual with complex health challenges. Pathways DC is one of the originators of the "Housing First" model to end homelessness, in which services are provided without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication. We believe housing is healthcare and that housing is a basic human right. Pathways provides an array of services including housing, street outreach, case management, drop-in services, and behavioral health support. Our model is based on client choice and the idea that the people we serve are the architects of their recovery!

In collaboration with Psychiatrists, nurses, social workers, and peers, the Substance Use Specialist provides recovery supports for people struggling with substance use in a compassionate and responsible manner. Integrated dual disorder treatment is provided using a harm reduction approach. Interventions include both group and individual therapy, relapse prevention, and education. The majority of the work is in people's homes and in the community.

Successful Substance Use Specialists are flexible and patient, work well in a fast-paced environment, and are independent thinkers.

RESPONSIBILITIES :

Provide outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, apartments, office, etc.;Collaborate with client in the formulation of an Individual Recovery Plan (IRP) that is reviewed and modified with client on a regular basis;Provide individually tailored services to each client in the following areas: housing, school, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, money management & entitlements, medication support, self-help & empowerment, problem solving, family life & social relationships and integrated treatment for substance use;Maintain written and computerized records compile reports and complete other program documentation (e.g., progress notes, incident reports, on-call logs, letters, and psychosocial assessments);Coordinate and monitor referrals to community services and advocate client participation;Lead groups, workshops, and in-service meetings on topics relevant to identified areas of client need;80% of time will be spent in the field/community, directly providing services to persons living with serious mental illnesses;Perform related work as assigned.QUALIFICATIONS :

DC Licensure as a Certified Addiction SpecialistMinimum of one year experience in social services, supported housing, or with persons with disabilities;Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations;Demonstrated knowledge of harm reduction theory/applications;Excellent communication, writing, and computer skills;Bilingual English/Spanish a plus;Driver's license and vehicle required;COVID-19 vaccination
