Job Description

PURPOSE OF POSITION

The Licensed Social Work Level Ill is an LCSW who as part of the SUD Services ED Leads Addiction Program, which is a mayoral initiative. The LCSW is part of the ambulatory substance use disorder (SUD) program at Kings County. The clinician is assigned to focus on connections to SUD care including work in the Emergency Department between Tours 2 and 3 as part of an interdisciplinary team. The LCSW functions as a Licensed Independent Practitioner who is the primary evaluator of adults presenting for SUD evaluation and treatment at multiple points of entry including the medical emergency room.

AREAS OF RESPONSIBILITY SUD Services Including:• ED Additions Leads Team• Addiction Consult Service• E-Consults• Centralized Intake and CDTOPS Program• Licensed clinician with 3+ years of clinical experience, supervisory experience and knowledge of substance use disorders, mental illness and co-morbid medical conditions.• Clinician should have familiarity providing assessments and interventions to individuals with history and current use of substance use.• The clinician is part of an ambulatory program including work in the emergency department.• They are responsible for working collaboratively with other disciplines and identifying the appropriate level of care for adult patients presenting for care and supervising and completing performance evaluations for licensed and non-licensed employees.• Candidate must be able to work with the severely mentally ill, substance abuse and intellectually disabled population, with knowledge of resources and services for this population.• The required shift to work is alternating between Tour Il and Ill shift and one day of the weekend.• They must be able to function as a Licensed Independent Practitioner who specializes in understanding the impact of addictions on adults presenting with SUD-related problems, including the emergency department and inpatient services, and actively linking them to treatment.

SUMMARY OF DUTIES AND RESPONSIBILITIES• The duties include, supervising direct reports, conducting initial social work assessment of patients presenting with SUD-related problems to multiple points of entry, including the emergency room, engaging in discussion around patterns related to use of substances and determining the most appropriate level of care to support them in their recovery.• The clinician should be skilled at facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, risk assessment, providing crisis interventions to high-risk patients, engaging with and providing support to families, and engaging them as partners in recovery.• Additional responsibilities include identifying and collaborating with existing support systems, facilitating community referrals as needed, collaborating with interdisciplinary treatment team to develop most appropriate case disposition, documenting all interventions within the required regulatory timeframes.• The clinician will be responsible for conducting annual performance appraisals of direct reports and managing case assignments. Good writing skills and ability to work in a fast-paced environment required.• English proficient a must; ability to communicate in Spanish or Creole a plus.

Minimum Qualifications

For Assignment to Level III1. Valid license as an LCSW issued by the NYSED.2. Master's degree in Social Work

Knowledge of Mental Health, Substance use disorders and service delivery to adults3. A minimum of 3 years' experience working with the substance use, mentally ill, medically compromised and intellectually disabled population.
