MAJOR DUTIES:

The Lead Case Manager will assist the Program Director in certain administrative responsibilities as directed and carry-on assigned CM duties. LCM providing support to homeless individuals or families. Responsibilities include client assessment and planning, resource coordination, advocacy, crisis intervention, documentation, staff training, collaboration with community partners, and ensuring quality service delivery.

BRIEF

DESCRIPTION :

Client Assessment and Planning: Conduct thorough assessments of clients' needs, strengths, and goals. Develop individualized service plans in collaboration with clients, focusing on housing stability, employment, healthcare, and other relevant areas.

Resource Coordination: Identify and coordinate access to housing resources, social services, mental health support, substance abuse treatment, and other community resources to meet clients' needs and promote stability.

Advocacy and Referrals: Advocate on behalf of clients to ensure they have access to necessary services and entitlements. Make appropriate referrals to external agencies and organizations for specialized support as needed.

Crisis Intervention: Provide crisis intervention and support to clients facing immediate challenges such as eviction, substance abuse issues, mental health crises, or domestic violence situations.

Documentation and Reporting: Maintain accurate and up-to-date client records, including case notes, assessments, service plans, and progress reports. Ensure compliance with program requirements and funding regulations. LCM will be responsible for maintaining their own case load and provide additional caseload coverage as needed during staff shortages.

Staff Training and Development: LCM will be responsible for training new case managers, enhance their skills in client engagement, assessment, service planning, and resource coordination.

Collaboration and Networking: Establish and maintain partnerships with other service providers, government agencies, housing authorities, and community organizations to enhance the range and quality of services available to clients.

Quality Assurance: Assist with Monitor the quality and effectiveness of case management services through regular review. Implement improvements as needed to enhance outcomes for clients.

MINIMUM QUALIFICATIONS:

Bachelor's or Master's degree in social work, psychology, counseling, or a related field. Previous experience in case management, preferably in homeless services or a related field. Strong leadership, communication, and interpersonal skills. Knowledge of community resources and social service agencies. Ability to work effectively with individuals from diverse backgrounds and cultures. Understanding of trauma-informed care principles. Commitment to social justice and advocacy for marginalized populations. Must be able to work with sensitively and effectively with diverse educational, socio-economic and cultural backgrounds
