Job Description

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Licensed Master Social Worker (LMSW) / CASAC based out of our busy Peekskill, NY health center.

This position provides crisis intervention, biopsychosocial assessments, individual, family and group counseling and/or case management services to HIV positive patients, their partners, caregivers and family members, as well as other patients with issues related to chronic illness, aging, maternity, domestic violence or other social problems. This position also provides education and counseling to patients and their significant others in need of addiction treatment counseling services. Performs job duties in compliance with systems and job specific standards.

ESSENTIAL FUNCTIONSPerforms a mental status exam on all new Mental Health patients and reviews with psychiatrist/primary care provider.Provides a comprehensive mental health intake/assessment for all new patients referred by primary care provider or other service provider for medical and/or social/emotional issues.Maintains data and case records as required and prepares necessary reports.Provides individual, group and family counseling to patients and their families.Provides creative prevention services to individuals at risk of infection and other chronic medical conditions or negative health or social factors.Provides case management services as needed to support primary care.Responsible for the accurate and timely completion of clinical documentation in patient records.Provides crisis intervention/emergency evaluations to patients when required.Required Education & Skills:

Masters Degree in Social Work (MSW)Bilingual English and SpanishActive NYS LMSW licensureActive OASAS Program CASAC (Credentialed Alcoholism & Substance Abuse Counsel)Required experience:

Salary: $65,138-$80,000 annual exempt for LMSW

Sun River Health is currently seeking a full time Psychotherapist to support our A28 Peekskill location.

POSITION SUMMARY

The Psychotherapist provides specialized knowledge and skills for psychiatric and mental health evaluation, assessment and management of clients. Strong therapy skills are essential. The position reports to the Senior Vice President of Behavioral Health

ESSENTIAL FUNCTIONS

Provides psychotherapy for patients.Provides psychosocial evaluations for patientsPractices in collaboration with psych NPs, psychiatrist and other medical staff to manage patient careAssesses the multidimensional needs of patients and develops and implements care plansFunctions as a patient advocate, by participating in clinical meetings, patient care conferences and informal discussion with health care and mental health program staffProvides patient education relative to psychiatric mental health issuesProvides written reports and care coordination as needed.Attends and participates in continuing educational programs, seminars, etc., designated to maintain professional competencePerforms all related duties, as assigned, or unrelated duties, as requested, during emergenciesDemonstrates an understanding of the management and care of the adult substance abuse population

EDUCATION/EXPERIENCE:

Master's Degree in Social Work or Mental Health Counseling

NYS licensure LCSW, LMSW or LMHC

At least 2 years' experience in the psychiatric mental health care of persons with HIV/AIDS.

Bilingual English/Spanish preferred

Salary Range: $65,000 - $100,000 annually based on licensure
