Job Description

TOUR II (8am -4pm)

Purpose of Position:

To provide proper linkage of patients discharged from the Woodhull CPEP to appropriate agencies in community for continuity of care and services.

Areas of Responsibilities:

Supervision and management of Bridger Program by visiting the patient in the community.

Summary of Duties and Responsibilities:

Under supervision, provides social work services and the coordination of continuing care planning, utilizing a variety of techniques such as individual, family, group work, and community networking to patients serviced at this facility.

• Conducts interviews with patient and/or family member(s)/significant other(s) to determine their continuing care needs. Conducts crisis intervention with high-risk cases presenting to the CPEP program. Completes Social Work Screening and Assessment for all the patients as outlined in departmental policy using the EPIC system.• Provides supportive counseling and crisis intervention, to patients and their families/ significant others as appropriate.• Devises/implements a social work treatment/continuing care plan which involves the patient, family/significant other and the interdisciplinary team.• Provides supervision and coordination of Peer Bridger program in consultation with the treatment teams.• Provides successful linkage of patients discharged from CPEP with community agencies and services.• Provides information and referral to community agencies, health homes.• Liaisons with patient and Health Care team to ensure patient understand his/her medical condition and disposition.• Advocates and liaisons for patients with community agencies.• Assists patients to implement post discharge Safety Plan of patients identified as High risk and Moderate risk for suicide in the CPEP.• Documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies.• Assists patients who need domicile placement with placement in appropriate shelter setting.• Initiates home visits in the community to engage and to support discharged patients from the CPEP and reconnect to the community providers and provide crisis intervention and management if needed.• Maintains and completes statistics of case activities as outlined in the departmental policy using EPIC.• Documents interventions in the EPIC medical record system.• Attends individual, unit, departmental and intra/inter hospital meetings as mandated by the supervisor and/ or department's director.• Assists /Participates in presentations, training, committees, special studies, research and in-services as required.• Participates in the departments cross-training and understands all aspects of discharge planning from an inpatient Psych unit working in conjunction with the departmental discharge planning unit.• Participates with other members of the CPEP team to optimize services to all patients.• Reinstates home care and visiting nurses' services from the emergency room as needed.• Secure an aftercare appointment for all discharged patients on afterhours, weekends and holidays on the following working day and communicate effectively with patients and families.• Provides supervisory coverage during the absence of the social worker level V due to annual or sick leave.SKILLS:• Interviews patient/family members/significant others in order to obtain pertinent psychosocial information and develop an appropriate social work/discharge plan.• Analyzes situations, identifies problems, evaluates solutions and implements an appropriate course of action based on the psychosocial data.• Knowledge of social work techniques for working with individuals, groups and families.• Communicates effectively with patient/family members/significant others, supervisor and the interdisciplinary team.• Documents all interventions and continuing care plans in the patient's medical record in clear, concise and legible manner.• Establishes priorities and accomplish multiple tasks.• Understanding the impact of the disease process on psychosocial functioning.• Community clinical follow up for the patients who are discharged from CPEP to confirm.Compliance with outpatient appointments.

Minimum Qualifications

For Assignment to Level III1. Valid license as an LCSW issued by the NYSED.
