Community Integration Counselor

to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community. This is a fantastic opportunity for professional growth, while making a difference!

No experience required– we provide PAID training!Starting pay rate is:

$17.70 an hourShift schedule:

Monday- Friday 8am- 4pmResponsibilitiesAssist individuals with daily living skills and the development of community involvement, communication skills, problem-solving skills, and social skillsProvide ongoing support for community integration effortsAssist with developing a schedule for each participant to maximize community integrationProvide training to individuals towards achieving program goalsComplete training to facilitate staff developmentAdminister medications and ensure proper completion of necessary documentationPlan, develop, facilitate, and participate in individual and group activities with participants,Including leisure and volunteer opportunitiesDevelop individual day program support plansProvide reports on individual progress in relation to overall goalsMaintain inventory and ensures adequate supplies for programOversee consumer funds as neededMonitor the health and medical needs of individuals and immediately report any concernsDrive individuals to medical appointments and activities in the communityEnsure program documentation and billable records are completed accurately and timelyBuild a support network for individuals by developing and maintaining relationships with community members, businesses, and familiesEmployees must cooperate with the licensee and department staff in any inspection, inquiry or investigationAdditional tasks and responsibilities may be assignedMinimum RequirementsHigh School Diploma or GEDBachelor’s Degree preferredValid driver’s license with a satisfactory driving recordMinimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilitiesAbility to establish a comfortable and supportive relationship with individuals receiving supportsAbility to exercise good judgment and remain calm in crisis situationsExcellent verbal and written communication skillsExcellent time management skillsWorking ConditionsTransport individuals utilizing your own vehicle or company provided vehiclesSchedule may change due to business needs and may include evening and weekend hoursMay be required to be on-call in cooperation with other management staffFrequent lifting, stretching, and other physical exertion may be requiredMay be required to lift or move 25+ poundsMay be exposed to various medical conditions and communicable diseasesWhy Community Options?Insurance Options (Medical, Dental, Vision)Paid Holidays—Including a Birthday HolidayGenerous Paid Time Off (PTO)Employee Incentive & Discount Programs403b Retirement Plan with Employer MatchExceptional Career Growth OpportunitiesFor more information, please call us at:
