Community Integration Counselors

, MD . The Community Integration Counselor (CIC) is responsible for assisting individuals with learningphysical, intellectual, emotional, and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community

Responsibilities

Support and encourage individuals to increase their ability to be as independent as possibleMeet with waiver participants on a regular basis either in the individual’s residence, the community, or an office settingCollaborate with the case manager to develop and facilitate programs which meet the social, emotional, physical, and personal needs of the individualProvide ongoing support for community integration effortsDocument individual progress toward acquiring community integrationAssist in building a support network between the individual and the communityEnsure medical consultation forms are completed, by the physician, for each medical appointment attendedAdminister medications and ensure proper completion of necessary documentationCoordinate all leisure and volunteer opportunitiesMonitor the health and medical needs of individuals and immediately report any concernsDrive individuals to medical appointments and activities in the communityEnsure program documentation and billable records are completed accurately and timelyAdditional tasks and responsibilities may be assignedEmployee must cooperate with the license and department staff in any inspection, inquiry, or investigationEmployment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testingMinimum Requirements

High school diploma or GEDBachelor’s degree preferredValid driver’s license with a satisfactory driving recordMinimum of two years of relevant experience supporting individuals with intellectual and developmental disabilitiesAbility to establish a comfortable and supportive relationship with individuals receiving supportsAbility to exercise good judgment and remain calm in crisis situationsExcellent verbal and written communication skillsExcellent time management skillsWorking Conditions

Transport individuals utilizing own vehicle or company provided vehiclesSchedule may change due to business needs and may include evening and weekend hoursMay be required to be on-call in cooperation with other management staffFrequent lifting, stretching, and other physical exertion may be requiredMay be required to lift or move 25+ poundsMay be exposed to various medical conditions and communicable diseasesWhy Community Options?
