NYC Health + Hospitals/Coler is an 815-bed nursing facility with a 5-Star CMS Quality Rating located on Roosevelt Island, nestled between Manhattan and Queens. Coler offers long term and subacute care, rehabilitation, and a dedicated memory care unit, comprised of 150 beds featuring an acclaimed music and memory program. The facility features a robust rehabilitation therapy program with expansive gym space for residents. Coler also offers full time physician staff, psychiatry, dental services, an onsite 8-chair dialysis den for hemodialysis, and on-site medical clinics to meet the clinical needs of its residents.

Job Description
Under general supervision with wide latitude for the exercise of independent judgment:
Provides medical; social work services to patients/residents. Applies casework and counseling methods to help individual patients/residents and their families when indicated.Provides group services to patients/residents and/or friends in areas assigned.Performs complex and difficult activity in social work practice, including psychosocial diagnosis, treatment, planning and implementation in collaboration with other disciplinesParticipates in unit team meetings and multidisciplinary meetings to discuss patient/resident care plan and discharge planning.Responsible for discharge planning including discharge follow-upMaintains up-to-date knowledge of community resources that may be needed for patient/resident care services and discharge planning.Maintains case records, activity statistics and prepares necessary social work reports.Coordinates and conducts Performance Improvement activities within the department regarding social work practice on complex issues.Acts as project leader for special studies and research projectsTrains and instructs social work staff in clinical and social work techniquesOrients and educates members of other professional disciplines on social work concepts and functionsAdditional tasks and functions as needed by the Director
Minimum Qualifications
For Assignment to Level II1. Valid license as an LMSW issued by the NYSED; or2. Valid license as an LCSW issued by the NYSED.
