Under supervision and within State, Federal, and County; develop programs and policies, identify client needs, funding sources, create and develop a service plan, implement, monitor and evaluate the service plan.Minimum Qualifications

1. Investigate reports sent from the Minnesota Adult Abuse Reporting Center (MAARC) with regard to alleged abuse, neglect or financial exploitation of Vulnerable Adults, and concurrently entering all investigative activities into a computer data base used by multiple staff.2. Interview reporters and collators to gather critical information to assess each report to determine the Vulnerable Adult status of the alleged victim, the presence of immediate danger to the Vulnerable Adult or others, the nature and severity of the alleged maltreatment, and the existence of a support system.3. Mobilize community resources to provide protective services to remedy the reported maltreatment and reduce the risk of further abuse, neglect, or financial exploitation.4. Notify Law Enforcement of any suspected criminal activity or suspicious death of a Vulnerable Adult that has not already been reported to law enforcement by MAARC.5.Take action on reports which require immediate adult protective services and initiate.6. Coordinate data sharing with Law Enforcement and Lead Agencies.7. Distribute relevant information to case managers and contract managers as appropriate.8. Create and maintain records.9. Communicate with interested parties and participate in assessment.10. Conduct forensic interviews using approved protocols.11. Collaborate with the various professions/agencies that may be encountered during an investigation such as medical and mental health, law enforcement, attorneys, courts and other social services staff, etc.12. Effectively engage individuals to assess and evaluate needs, strengths, safety and risk while promoting autonomy and least restrictive interventions.13. Formulate case plans to determine appropriate services to address client needs, including emergency interventions and protective services.14. Provide time limited, intensive case management services to vulnerable adults with complex cognitive, medical and psychiatric disorders.15. Apply knowledge of relevant policies, procedures , statutes and professional practices16. Complete agency, state and federally required documentation as related to client activity according to mandated timelines.17. Ability to work flexible schedule in response to emergency case assignments, clients availability and other work related duties that may result in working outside the normal business hours.18. Ability to present to the general public about adult protection statutes and practices specific to Dakota County.19. Testify in court, prepare court documents and work with individuals, families and professionals involved in the court process.20. WORK ENVIRONMENT: Work is performed in a County office building and on visits to client's homes. Some travel may be required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to hazardous physical conditions such as mechanical parts, electrical currents and vibration; atmospheric conditions such as fumes, odors, dusts, gases and poor ventilation; hazardous materials such as chemicals, blood, and other body fluids; inadequate lighting; work space restricting movement; and intense noise may occur on visits to client's homes. Equipment used may include, but is not limited to, computer, printer, telephone, calculator, and copy and fax machines.
