Position : Licensed Master Social Worker (LMSW)Location : New York, NY 10038Shift : 8:30 AM - 4:30 PM (includes a 30-minute break)Department : Care Coordination and Social WorkJob Summary :
This is a 59-day assignment requiring a Monday to Friday schedule, with possible holiday shifts. The Licensed Master Social Worker will provide vital social work services within a hospital setting, focusing on case management and discharge planning.Responsibilities :Conduct comprehensive assessments to determine patients' social and emotional needs.Develop and implement effective discharge plans, collaborating with the interdisciplinary care team.Manage a caseload efficiently, demonstrating robust multitasking abilities.Utilize EPIC EMR for patient management and documentation.Handle Allscripts for referral management and coordinate transportation setups as needed.Interact and collaborate with various healthcare professionals to ensure comprehensive patient care.Support psychiatric and pediatric populations, addressing their unique needs.Qualifications :Master's Degree in Social Work (MSW) (required).Licensed Master Social Worker (LMSW) by the State of New York (required).Previous hospital social work experience (required).Case management experience with a focus on discharge planning (preferred).Strong computer navigation skills and familiarity with EPIC EMR and Allscripts (preferred).
