Behavioral Health Integration Clinician IDepartment: Behavioral Health Integrations and Primary CareLocation: Swedish Hospital Pro Plaza. 2740 W. Foster Ave, Chicago ILHours: Generally 8am-4:30pm with some flexibility in start/end timeDays: Monday thru FridayShift: Day ShiftPart time/Full Time: Full Time 40 hours per weekWhat you will need:Master's Degree or higher in a related mental health fieldCurrent State of Illinois licenses such as: Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) within six months of start date into positionCPRIllinois Driver's LicensePreferred a minimum of one year or more of experience in mental health setting or healthcare setting.General Summary:Reporting to the Clinical Manager or designee, with accountability to the care team, provides basic nursing and personal care to patients as delegated by and under the direction of a Registered Nurse and Licensed Practicing Nurse.
