URPOSE:

The Social Worker is a member of the Intensive Foster Care Program team serving children and youth who are in the custody of the Department of Children and Families. The Social Worker provides direct clinical and case management services to children in foster care placement, provides support to foster families in coordination with the IFC Family Resource Worker, and support's the youth and their team on locating and strengthening permanent connection.The Agency:

Key Job Tasks:

Completes weekly home visits with children in foster homes to ensure safety, monitor progress and support placement

Utilizes permanency interventions with youth, families and DCF in order to enhance permanent outcomes

Provides Case Management and treatment planning for all children on caseload.

Providers after hours emergency coverage on a rotating basis

Education & Experience Required

Bachelors degree in social work, psychology, education, or a related field plus 1 year experience working with children, youth and families or MSW

Professional Social Work licensure (within 6 months of hire).

Bilingual preferred but not required

Valid driver's license and properly insured vehicle

Fully vaccinated required.
